Preparing for Your Office Visit

When scheduling an appointment, please be prepared to provide the reason, or chief complaint, for your appointment.

Please kindly provide at least 24-hour notice if you are unable to keep your scheduled appointment so that other patients may receive medical care.

Please arrive 15 minutes before your scheduled appointment to allow ample time to complete paperwork at check-in. One of our medical assistants will then take you to an exam room where pertinent medical information will be updated. Patients arriving more than 10 minutes after their scheduled appointment time may be asked to reschedule their appointment.

Please bring a list of your medical history and medication (with strength and dosing instruction). Please include any vitamins, supplements or herbal medication you are taking. If you are an established patient, then please be prepared to provide any updated medical history or new medications.

We ask all new and established patients to present their photo identification and insurance care at every office visit. Patients should also anticipate paying required co-pays at check-in. As a courtesy, we bill every patient’s insurance. Any remaining balance after health insurance benefits are applied will be billed to the patient. We kindly ask that balances be paid within 30 days.
